What Are the Symptoms of Hormone Imbalance?

Hormones are chemical messengers that regulate virtually every function in your body — energy, mood, weight, sleep, libido, cognition, immune function, digestion, and more. When they’re in balance, you barely notice them. When they’re not, the effects can be far-reaching, deeply uncomfortable, and surprisingly hard to pin down.


Hormonal imbalance is one of the most common and most commonly missed drivers of chronic symptoms — in both men and women, across a wide range of ages. This page covers the key hormones, what happens when they go out of balance, and how to know whether hormonal assessment might be worth pursuing.

Thyroid hormones (T3 and T4)

Thyroid hormones regulate metabolism at the cellular level throughout your entire body. They affect how quickly you burn calories, how efficiently your cells produce energy, your heart rate, body temperature, digestion, and much more. Thyroid imbalance — in either direction — produces wide-ranging symptoms that are easy to attribute to other causes.  We also prescribe T2 and immediate and sustained release T3/T4 combo therapy compounded.  

 

Signs of low thyroid (hypothyroidism): fatigue, weight gain, cold intolerance, dry skin and hair, hair loss, constipation, brain fog, depression, slow heart rate, fluid retention, joint pain.

 

Signs of high thyroid (hyperthyroidism): weight loss, rapid heartbeat, anxiety, tremors, heat intolerance, excessive sweating, insomnia, diarrhea.

Cortisol

Cortisol is your primary stress hormone, produced by the adrenal glands. It regulates your sleep-wake cycle, blood sugar, immune response, and energy levels. Imbalanced cortisol — too high, too low, or dysregulated in its daily pattern — produces symptoms that can be difficult to distinguish from other conditions.

Signs of high cortisol: weight gain (especially abdominal), sleep disturbances, anxiety, high blood pressure, blood sugar instability, increased susceptibility to illness, bone loss.

Signs of low or dysregulated cortisol: profound fatigue (especially morning fatigue), difficulty handling stress, low blood pressure, salt cravings, poor immune function, brain fog.

Estrogen and progesterone

In women, the balance between estrogen and progesterone governs the menstrual cycle, fertility, mood, sleep, and much more. Imbalances between these two hormones — particularly the pattern known as estrogen dominance — produce a distinct cluster of symptoms.

Signs of estrogen dominance / low progesterone: irregular or heavy periods, PMS, mood swings and irritability, breast tenderness, bloating, fluid retention, weight gain (particularly hips and thighs), sleep disturbances, anxiety, headaches.

Signs of low estrogen (common in perimenopause and menopause): hot flashes, night sweats, vaginal dryness, brain fog, mood changes, sleep disruption, bone loss, joint pain, dry skin.

Note: Estrogen balance is also relevant in men, where excess estrogen relative to testosterone produces fatigue, weight gain, reduced libido, emotional sensitivity, and other symptoms.

Testosterone

Testosterone is commonly thought of as a male hormone, but it plays important roles in both sexes — affecting energy, muscle mass, libido, mood, cognition, and metabolic health. Testosterone declines with age in both men and women, but can drop significantly in earlier adulthood due to stress, poor sleep, nutritional deficiency, and other factors.

Signs of low testosterone in men: fatigue, reduced libido, erectile dysfunction, loss of muscle mass, increased body fat (especially abdominal), depression, irritability, brain fog, reduced motivation.

Signs of low testosterone in women: fatigue, reduced libido, decreased motivation, difficulty building muscle, weight gain, brain fog, mood changes.

Insulin

Insulin regulates blood sugar and fat storage. Insulin resistance — where cells become less responsive to insulin’s signals — is one of the most prevalent and most consequential hormonal imbalances, and one that develops silently over years before producing a diagnosable condition.

Signs of insulin resistance: weight gain (especially abdominal), sugar and carbohydrate cravings, energy crashes after meals, difficulty losing weight, fatigue, brain fog, darkened skin patches (acanthosis nigricans), and over time, elevated fasting glucose or triglycerides.

DHEA

DHEA (dehydroepiandrosterone) is an adrenal hormone that serves as a precursor to sex hormones and plays a role in energy, immune function, and mood. It declines significantly with age and is often low in people with adrenal dysregulation or chronic illness.

Signs of low DHEA: fatigue, decreased libido, reduced resilience to stress, depression, decreased immune function, and accelerated aging.

What Are Symptoms That Frequently Signal Hormonal Imbalance?

Because multiple hormones interact with each other, hormonal imbalance rarely produces a clean, single-system picture. The following symptoms, particularly when they occur together or without an obvious cause, are frequently associated with underlying hormonal dysregulation:

  • Persistent fatigue despite adequate sleep

  • Unexplained weight gain or inability to lose weight

  • Brain fog, poor concentration, or memory problems

  • Mood changes — anxiety, depression, irritability, or emotional instability

  • Sleep disturbances — difficulty falling asleep, waking in the night, or unrefreshing sleep

  • Changes in libido

  • Hair thinning or loss

  • Dry skin or changes in skin quality

  • Irregular, heavy, or painful periods

  • Hot flashes or night sweats

  • Cold intolerance or always feeling cold

  • Digestive changes — constipation, bloating, or irregularity

  • Muscle weakness or loss of muscle mass

  • Joint pain or stiffness

  • Frequent illness or slow recovery

How Is Hormonal Imbalance Diagnosed?

A thorough hormonal assessment goes well beyond the basic panels included in most annual physicals. Comprehensive evaluation typically includes thyroid (TSH, free T3, free T4, reverse T3, thyroid antibodies), sex hormones (estradiol, progesterone, testosterone — total and free, SHBG), adrenal hormones (cortisol curve, DHEA-S), insulin and blood sugar markers, and often additional hormones such as leptin and IGF-1.
 

Equally important is interpreting results against functional optimal ranges, not just conventional reference ranges — because many hormonal imbalances produce significant symptoms at levels that are technically “normal” by population standards.
